All Individual Players must have the following (3) three forms (Red Boxes), the PLAYER INFORMATION & RELEASE FORM, the GREAT WESTERN WAIVER, and the US LACROSSE WAIVER...
These 3 forms must be filled out by all players participating in the Tournament. Used by both MS & HS Players...
