Three forms for each player and coach
participating (Red Boxes),
Pay online by credit card
(Gold Box),
and go back
to the info pages
(Blue Boxes).
Hit your back button on your browser and repeat the process for all
of the required Tournament forms.
The forms you will need
as a player or coach are the PLAYER
INFORMATION FORM, the
GREAT WESTERN WAIVER, the
US LACROSSE WAIVER.
The team and coach will need the
TEAM APPLICATION & TEAM ROSTER FORM...
