REGISTER A
TOURNAMENT TEAM
To register a team and secure a space in
the tournament, you
will need to print a TEAM APPLICATION, fill it out and
mail it,
along with a non-refundable check for $325.00, to the address
listed below. A confirmation Email will be sent to you when we receive the
application, along with the due dates for the
remaining paperwork and player fees. All the forms needed on
the next page in the tournament tour. Click
the gold box below.
STEP 1
Print the TEAM APPLICATION. Mail is to the address provided along with a
non-refundable deposit of $325.00, made out the
Great Western Lacrosse.
DUE DATE
As soon as possible. We only take 8 teams and do so on a first come, first
serve basis.
STEP 2
Print the TEAM ROSTER and the
US LACROSSE INSURANCE ROSTER FORMS. Form your team and fill them out. Mail
them to the address provided.

JULY 1st, 2010
STEP 3
Secure your team uniforms.
Please note that you can have any color jersey that you wish except for the
following colors:
WHITE, GOLD, GRAY & ORANGE. When you know your Jersey color, please let the
Tournament Director know.

STEP 4
Print the PLAYER APPLICATION &
RELEASE, the TOURNAMENT COMBINE WAIVER and the TOURNAMENT SWIM WAIVER FORMS.
Make copies of all three forms for each of your team players and distribute
them so parents can fill them out. Then collect these three forms from each
of your players along with a check for $325.00, made out to Great Western
Lacrosse. Mail all forms
and check to the address provided.

STEP 5
Secure transportation to the
tournament. Arrange your airfare if your team is flying and arrange your
ground transportation either through us or on your own.
